Are you looking to understand why a unique live liquid bacteria. The HealthPathways Community is a collaborative of all organisations implementing HealthPathways. Each organisation undertakes to share knowledge, processes, and pathways with other members of the Community.

The Community is facilitated by Streamliners, a New Zealand based technical writing and publishing company. Health Pathways designs digital solutions for clinical pathways. Beautifully simple and easy to access, our clinically driven products are flexible, adaptable and can track patients across organisations, from referral to follow up improving information sharing through a digital end-to-end process.

What is HealthPathways ? HealthPathways offers clinicians locally agreed information to make the right decisions together with patients, at the point of care. The pathways are designed primarily for general practice teams, but are also available to specialists, allied health professionals, and other health professionals in your region. HealthPathways is a new digital platform offering referral pathway information that has been written for clinicians, by clinicians. When someone visits their GP with an issue, they may be referred to specialist care in a secondary care setting.

New pathways have recently been published on the Mid and North Coast HealthPathways website. New HealthPathways Adverse Events Following Immunisation (AEFIs) Adult Sepsis (Including Meningitis) Fever in Children GP Eating Disorders Treatment Plan Neuropsychology Assessment Reviewed and Updated HealthPathways Non-healing Wounds Child or Young Person at Risk Screening for Fetal Anomalies. HealthPathways is web-based information portal supporting primary care clinicians to plan patient care through primary, community and secondary health care systems within Mid and North Coast. HealthPathways Melbourne is a free, web-based portal with relevant and evidence-based information on the assessment and management of common clinical conditions including referral guidance. Healthpathways Progress is well underway to localise hundreds of clinical pathways which, when complete, will be used within general practice to assist with the care and management of patients within South Tyneside.

HealthPathways is an online manual used by clinicians to help make assessment, management, and specialist request decisions for over 5conditions. Rather than being traditional guidelines, each pathway is an agreement between primary and specialist services on how patients with particular conditions will be managed in the local context. HealthPathways are designed to be used at the point of care, primarily for general practitioners but is also available to hospital specialists, nurses, allied health and other health professionals.

SpotOnHealth HealthPathways is not designed for patient use, therefore a username and password is only provided to clinicians working in the Brisbane South region, including Brisbane South, Redlands, Logan and the Scenic Rim catchments. HealthPathways Developed by local GPs, specialists, nurses and allied health providers, Western Sydney HealthPathways supports health professionals by providing local, relevant information on managing medical conditions and referral options for their patients. HealthPathwaysis an online portal that provides general practitioners and other health professionals with easy access to comprehensive, evidence-based assessment, management and localised referral resources for specific health conditions. HealthPathways WA is web-based information on referral and management pathways helping clinicians to navigate the patient through our complex primary, community and acute health care system within Western Australia. The aim of HealthPathways is to improve the health experience and shorten the time taken for people accessing primary health care and hospital services.

The essence of HealthPathways is collaboration between GPs and Hospital clinicians that in published local agreements on best practice. The SES HealthPathways website is available to local health and social care service providers including local general practitioners, allied health professionals, RACFs, local government and non-government organisations, and South Eastern Sydney Local Health District clinical staff. The secure portal is not for use by patients, however there will be appropriate local resources for GPs to provide to patients.

HealthPathways Sydney is an online local health information portal to support local GPs and health professionals at the point of consultation. It provides clinical decision support frameworks on how to assess and manage medical conditions, and how to appropriately refer patients to local services and specialists in the most efficient way.
